Having a good and healthy gums and teeth can make us smile and talk at ease. When a person loses his teeth due to an accident or congenital defect, he may be reluctant to smile and he may not be able to talk properly. I want to go into the field of dentistry so I can reach out to the indigent people who could not see a dentist due to lack of finances or dental insurance. I want them to know about the importance of person and professional care in the prevention of dental disorders. I also intend to participate in the dental mission and render my service to the less fortunate people who are living in developing countries. This is my way of sharing my knowledge and blessings to others. After completing the course in Dentistry, I intend to work and hone my skills as a dentist. He even attacked Hollywood for communism; in one case he ordered Charlie Chaplain out of the USA for un-American activities. The trials carried out by Joe McCarthy were very flawed and there were numerous appeals. The McCarthy period started when the Russians joined the ‘nuclear club’ sooner than expected, and caused paranoia across the USA. This is reflected in the Salem society because the population of Salem started to blame people for witch-craft to get themselves more respect and to settle grudges. This was then named the witch hunt. It is a puritan society that believes they themselves were chosen by God. They also believe they could only work and pray; that the Universe was centred on God; and that man was sinful and corrupt but thankfully saved by God. Their society was ruled by religion and was very rigid. This is ironic because the lies and trickery performed by the girls of the council goes against the Salem society, and yet they believe what they say. Reverend Parris of Salem was a high-ranking individual who demands a lot of respect that he thinks he deserves. He is dogmatic and dominant and thinks himself to be the top man of Salem. His niece Abigail is soon to follow him by being a snob and constantly lying. Abigail is attracted to John Proctor, a man that in the Salem society is a dangerous man and is to be afraid of. Reverend Parris’ maid Tituba is a black slave in her forty’s from the Caribbean. She is an innocent women but trouble always seems to follow her. Betty Parris (Reverend Paris’ Daughter) is thought to have been consumed by the devil; she was involved in an act of ‘conjuring’ and was then left unconscious, this is when the witch-hunt began. The play is set in the 1962 witch trials of Salem . This is when people were blamed for witch craft to settle grudges and rivalries. The trials were flawed and even the jury was lying. This is linked to the McCarthy period because Joe McCarthy blamed others for communism to settle his grudges and rivalries against him. Abigail Williams is the niece of Reverend Parris. Abigail Williams is a young â€Å"strikingly beautiful† seventeen year old girl who is an orphan of low social status. She once had an affair with John Proctor when she was his maid; she was then ordered out of their house after John Proctor’s wife, Elizabeth Proctor, discovered this. Abigail Williams has a grudge against Elizabeth Proctor. Abigail is an aggressive woman, â€Å"I’ll beat you Betty! † From this, the audience can tell how far Abigail will go to get out of trouble. In addition to this, we recognise how cruel and selfish she is. Furthermore, it shows that she doesn’t care about anyone other than herself; not even her cousin. Arthur Miller portrays Abigail Williams in this manner to shock the audience by presenting Abigail Williams as an outcast compared to all the other women in the village. When women are addressed in the Salem society, they are usually referred to as â€Å"Goody†; this isolates Abigail Williams because her reputation is â€Å"blackened in the village†. Abigail Williams has strong feelings for John Proctor after the events which happened when Abigail Williams was John Proctor’s maid, â€Å"Gah! I’d almost forgot how strong you are, John Proctor! † From this quote the audience can connote the deep feelings that Abigail Williams has for John Proctor. Use of the word â€Å"forgot† shows the audience that Abigail Williams and John Proctor had a past life together. This also outlines Abigail Williams grudge against Elizabeth Proctor. In addition, the stage direction â€Å"she comes a little closer, with a confidential, wicked air† shows the deep temptation that Abigail Williams has for John Proctor. Abigail Williams has made a confliction for herself between John Proctor and herself, â€Å"My wife is innocent, except she knew a whore when she saw one! † By use of the word â€Å"whore†, John Procter immediately shows the audience the disgust he holds for Abigail Williams for ruining his love life with Elizabeth Proctor. In later acts, John Proctor further shows the audience that he hates her for what she has done when he says â€Å"A whore’s vengeance†. By saying this John Proctor shows the audience how much his life has been ruined due to his affair with Abigail Williams. Abigail Williams holds strong grudges against Elizabeth Proctor, â€Å"It’s a bitter woman, a lying, cold†¦ † This quotation shows the outright hatred she has for Elizabeth Proctor. By referring to Elizabeth Proctor as â€Å"it† shows a lack of respect because she talks about Elizabeth Proctor as if she was an object. Furthermore, Arthur Miller uses the rule of three to emphasise her pure hatred against Elizabeth Proctor. The words that Abigail William’s uses sound cold and dirty, this emphasises her rivalry against Elizabeth Proctor because Abigail William’s is trying to make Elizabeth Proctor sound like she has no heart. When Abigail Williams is introduced, Arthur Miller makes us question her character, â€Å"with an endless capacity for dissembling. † This stage direction shows the audience that Abigail Williams has a bad reputation. The use off the word â€Å"endless† shows that she will stop at nothing to get hew own way; it also puts the audience at the edge of their seats. Abigail Williams also has a reputation for doing anything to get herself out of trouble, â€Å"Not I, Sir- Tituba and Ruth. † The audience recognises that Abigail Williams will lie to get herself out of trouble. This is linked to the McCarthy period where people would blame other’s to climb higher in the social ladder. Arthur Miller also introduces the fact that Abigail Williams is ruthless, aggressive and selfish to the people that surround her. This is reflected in modern day life because Joe McCarthy was also very ruthless to his grudges and rivalries. The girls of Salem are scared by Abigail Williams because she is wicked and sly to them, â€Å"Let either of you breathe a word, or the edge of a word, about the other things and I will come to you in the black of some terrible night and I will bring you pointy reckoning that will shudder you. † From this speech by Abigail Williams the audience can see Abigail Williams controlling the other girls that danced with her in the forest. By use of the word â€Å"black†, Abigail Williams tries to remind the other girls of the devil and scare them. In addition, â€Å"pointy reckoning† is used to scare the girls because it reminds them of the devil and they are extremely religious. Even the â€Å"der† in â€Å"shudder† provokes the feeling of endless pain. Abigail Williams developed her violent lifestyle when she was young, â€Å"I saw Indians smash my dear parents’ heads on the pillow next to mine, and I have seen some reddish work done at night. † From this the audience can sympathise with Abigail Williams because she had a troubled child hood. However, Abigail Williams uses this speech to terrify the girls that danced with her in the forest. The phrase â€Å"next to mine† emphasises the close shave of death that Abigail Williams has had; she introduces to the girls that she has seen someone killed and she knows what its like. Abigail Williams enhances the fact that she has no problems to kill the girls of Salem by saying â€Å"I have seen some reddish work done at night. † In addition, the personal pronoun, â€Å"I† underlines to the girls that she will do it herself. â€Å"Reddish† also enhances the gory and graphical image that Abigail Williams is trying to create inside the other girls heads to scare them further. He has worked very hard to get in his position and he wishes to keep it. However, from Reverend Parris’ high status, he has become very paranoid. This is reflected in the McCarthy period because Joe McCarthy became Senate of the USA and he also got very paranoid. Reverend Parris is obsessed with his reputation and strides for a high status within the Salem society,† he cut a villainous path. † This stage direction evokes the feeling that Reverend Parris is not entirely honest; the use of the word â€Å"cut† connotes that he some how cheated his way to this high ranking authority in the community. This reflects the time in which Arthur Miller was writing The Crucible because Joe McCarthy blamed people for communism to stay as the Senate of the USA. The Mishap 250 The Continental Airlines Flight 1404 was a passenger carrier flight from Denver International Airport at Denver in Colorado to George Bush Intercontinental Airport at Huston in Texas. On 20th December 2008, at 1818 Mountain Standard Time, the Continental Airlines flight 1404 (a Boeing 737-500, N18611) skidded off the left side of the runway while taking off from the Denver International Airport. As an impact of the skidding, the flight crashed into 40 feet deep ravine which was several hundred yards away from the runway. During the course of the crash, the flight caught fire. Why the Accident was Survivable While analyzing this disaster case, it seems that flight attendants’ timely responsive actions significantly contributed to the survivability of this aircraft crash. Although the flight attendants could not communicate with the pilots immediately after the aircraft came to a halt, they timely ordered an evacuation at the moment they identified the danger or fire. Since the right side of the airplane was almost caught by fire, three exits on the plane’s left side including forward, aft, and overwing were used for the evacuation process. When the forward and aft exits were operated by flight attendants, a passenger operated the left overwing exit. During the evacuation, three flight attendants and two other flight crewmembers assisted passengers to escape through less congested exits and blocked access to right side exits. In the opinion of Leib (2009), the accident was survivable because only left side of the aircraft caught fire and therefore passengers could escape through right side exits. The painting manifests three levels of viewing3. In the first instance, the viewers of the painting find themselves fixed in the position of the actual audience in the theatre. The second level now directly concerns the image and there is a woman displayed as one of the audience who is also watching the performance by the dancers. The third level depicts the actual performing dancers in the stage and of whom all the attention is geared. Edgar utilizes differ ent artistic styles in ensuring an effective visualization by the appreciators. There is effective harmonization of the light, color and shadows. All these attributes to the observable quality appreciated in the painting2. There is a distinction between the leader of the dance group who is in a light orange dress and is in front of the other dancers who are in green gowns with red stripes4. In Mary Cassatt’s, At the Opera the image also dominate a significant portion of the painting. The piece exhibits a woman seated alone in the balcony to observe performance in a theatre. The woman has a long black dress that covers her body properly from the lower part of her neck till the feet. The proper woman also has gloves on her hands and a black hat. These leaves only the face and few parts of the hands exposed for the public view. The woman has a pair of binoculars with which she uses to capture the performance explicitly in the hall4. Professional Development Plan Essay The group members of Learning Team D 3 took a DISC assessment test to determine their personality styles, strengths and weaknesses, and areas that need enhancement. The group members are (Antonia, Brenda, Matthew, Robin, Steven). The Professional Development Plan will give a description of the strengths, and weaknesses each personality styles, individualities each styles share, and characteristics that may cause conflict, and how each styles can work together for the betterment of the organization. As well as to show how I, the manager can improve my abilities to lead from each team member. The DISC assessment generated two personality types within the group: Cautious, and Dominance. Three out of the five members of the group including my-self are labeled as Cautious. The Cautious strengths entails independence, dependable, good listeners, ask numerous questions over critical information, and are good at follow-through, they want perfection, and tend to be tense with themselves and others when under pressure, they have a natural curiosity about people, and are concerned about what people expect, think, and feel, they have strong affections to their personal interests, undervaluing the time required to complete tasks, being intuitive and observant about situations and people, they tend to associate their self-worth with their work accomplishments, and are intrigued by concepts, ideas and processes. They are known for being assessors and tend to have strong opinions about people and groups with whom they do not agree or identify. Their weaknesses of Cautious are they tend to work at a slow pace, unlike The Dominance personality types who are goal-oriented, fast-paced, take authority, and willing to challenge outdated thinking and ideas. Dominance weaknesses are what make them stand out from Cautious personality type. Dominance is not afraid to bend the rules; they become annoyed with delays, and prefer to ask for forgiveness than ask for permission to make certain decision on assignments. In order for each group member to work together as a team; it is important to first understand each member’s strengths and weaknesses and utilize them as a tool for the group  to accomplish their goal. Evaluating the different personality types I’ve come to the conclusion that Dominance and Cautious types are similar in personality traits, they are both problem solvers, efficien t, productive, independent, and have the same sense of irritations. The only differences between these two types are the pace in which they choose to work. The Cautious style prefers to work at a slower pace whilst, the Dominance style prefers to work at faster pace. Subsequently, understanding each personality styles, the characteristics of each member, and what each group has in common. We now need to utilize each of these characteristics and use them as a tool to accomplish our goals. For example, if our group was working for a financial organization, the Cautious types would be best place in a Human Resources position or even in the Marketing department. Those positions require individuals that possess such traits as being detail oriented, independent workers, and follow-through on certain types of assignments. Dominance type would be greater in managerial positions, for they like to take charge, and are comfortable in a fast paced environment; they are willing to take on challenges head on, and don’t mind taking risks. In order for the organization to run smoothly all these positions need each other in order to become succes sful, and to work as a team. Even though my personality style is Cautious I feel a good leader is usually passionate about the organization and his or her work, exudes confidence in his or her abilities, can organizes and makes sense of complex situations, maintains high standards and inspires others to do the same, can motivate and inspire employees, and is generally looked up to as a person of vision (Hill, 2009). This is a quote I feel strongly about and would want to base my leadership style on. I agree with the whole statement it expresses, and feel I can be inspirational with my group and motivate them to accomplish our goals. As a leader, I look forward to learning the skills that are required in order to become a better leader/manager. The DISC assessment has shown the different characteristics each personality styles possess. There are numerous traits each type shared, and a few that could cause conflicts, we must learn that we are all different individuals who have different learning styles and traits, as a result, we much learn from each other in order to grow and become better individuals, and need to help each other to be better individuals whether in work or educational environment. It is imperative  that each group member have an understanding on how to rely on each other even though there is discomfort with the various types of personalities. My capability to lead the team will be based on my willingness to learn from each team member and to understand that we should work as a team and not as an independent person on a project. All of the information provided above plan will be supervised closely, with periodic meetings organized in order to allow employees to give their views, opinion and concerns about various topics, that way it will help to ensure that full compliance is being achieved. I believe it is imperative that regular (weekly) monitoring is performed to make sure that the organization stays on track on all assignments, and concerns are being discussed, this will help ensure the long term success of the organization, and recognizing each individual’s style, as well as their strengths, and weaknesses will permit a more personal and individualized approach taken in regards to team dynamics and goal achievement. Evidence Based Screening Case Study Christopher Mason Clinical scenario Miss. J, (not the patient’s real title or Initial), is a 37-year old female who was recently admitted to the hospital for chronic fatigue leading to exhaustion. As a career she says, â€Å"I am a professional writer and aspiring actress.† She lives alone on the Westside of Los Angeles in a one bedroom apartment. During our conversation she disclosed that after a five year period of not smoking; she has begun smoking again. Miss. J says that the stresses that come with her career path as an actress and writer along with the people involved in the same has most likely caused her to do so. Miss. J denies any illicit drug use but does occasionally use alcohol as a stress reducer. As far as she knows no one in her family has died from or developed any type of cancer. Her father passed away from Acute Myocardial Infarction and was a type 1 diabetic. Her mother has steroid induced type 2 diabetes. Miss. J says, â€Å"I have managed to avoid both. So far.† Identification of the Evidence Based Guideline Based on the interaction with Miss. J, several risk factors became apparent and they include a family history of diabetes, type 1 and type 2. Even with type 2 diabetes having a strong familial association her mother’s was induced by steroids rather than being genetic. The other risk factor that I identified was the smoking of cigarettes and casual use of alcohol as a stress reducer. This places her at an increased risk for use of other illicit drugs, driving accidents, lung cancer, COPD, and high blood pressure, as well as other smoking related diseases. Reason for Selecting Guideline Based on the United States Preventive Services Task Force (U. S. Preventive Services Task Force, 2010), being screened for high blood pressure is recommended as an â€Å"A† level screening. The current level defining hypertension in adults is equal to or greater than 140 for systolic and equal to or greater than 90 for diastolic. Based on variability between separate readings, it is recommended to obtain greater than 2 readings on different visits over several weeks to diagnose hypertension. (p. 35) Barriers and Facilitators for Client Miss. J has only one barrier that I could pinpoint to following up with the screening. Her fatigue. She told me that sometimes she misses appointments because she is too exhausted to drive to them. She agreed that she would make an appointment with her personal physician after being released from the hospital to be screened for hypertension. Adding that she just hoped that she would be able to make the appointments. One of the facilitators that allowed me to participate in a conversation with Miss. J concerning her hypertension was her stating to me that she wished she could have a smoke. That along with her being a captive audience in the hospital. No one had been to visit with her that I had seen, so I assumed and was correct that she would like to chat. Receptivity of Client Miss. J was receptive to the screenings I have listed below. She stated to me that she had recently had a pap smear and tested negative for abnormal cervical cells and HPV. Being asked about the Chlamydia screening because of being sexually active; she said it was something she was interested in receiving more information about. Evidence Based Screening Guidelines Pertinent to Patient Type Clinical Considerations and Currency Situation Cervical Cancer Women 21 to 65 (Pap smear) every 3 years or, for women age 30 to 65 years who want to lengthen the screening interval, screening with a combination of cytology and HPV testing every 5 years. Grade A+ Recommended. (Screening for Cervical Cancer, 2014). Yes, she is current. Chlamydia All sexually active non-pregnant young women aged 24 and younger and for older non-pregnant women who are at increased risk. Grade A+ Recommended. (Section 2. Tobacco Use This recommendation applies to adults 18 years or older and all pregnant women regardless of age. Healthcare Provider Barriers I am performing my clinical rotation at Cedars Sinai in Beverly Hills. They do not allow us to use electronics in or around the patients/clients. After explaining to my clinical liaison about our subject and assignment we were cleared to use the program as long as we did it outside of the patient’s rooms. This was limiting but not considered a complete barrier. Confidence in Providing Evidence Based Screening Guidelines This assignment made me feel uncomfortable at first. I realized that I would be asking some very personal questions to a patient that I did not know. However, it did allow me the chance to become familiar with the AHRQ evidence based guidelines application on the android market. The Ozone Ozone derives from the greek word ozein which means to smell. It was first discovered in 1839 by Christian Friedrick Schonbein who noticed it because of its distinctive acrid smell. He discovered this at the University of Basel in Switzerland. Ozone is merely oxygen, but not the type we breath. Ozone, O3 has three compounds while oxygen has only two. Ozone is reac- tive, meaning it does not stay still, and wants to go back to its original state, with two compounds, O2. This is why ozone isharmful. Ozone always wants to let go of its third compound, and if this compound reacts with other substances, it could be damaging, especially to humans. When discussing with the ozone layer, one should know the four major atmosphere levels on earth. The troposphere which is between zero and fifteen kilometers in altitude and has tempera- ture ranges from two hundred to two hundred ninety kelvins. The second is the stratosphere which ranges from fifteen to approxi- mately fifty kilometers in altit ude and has temperature ranges from two hundred to two hundred fifty kelvins. The third level in the atmosphere is mesosphere. This level ranges from fifty to eighty-five kilometers in altitude and has temperature rangesbetween one hundred eighty and two hundred fifty kelvins. Finally, the thermosphere is the final level in the atmosphere. It's range is eighty-five to one hundred forty kilometers and also temperatures as high as four hundred sixty kelvins. Society has been widely addressed with the many problems that we are having in our environment today. A major problem is that of CFCs. CFC stands for Chlorofluorocarbons which are found in many of the aerosol spray cans. In December of 1973, Rowland and Molina discovered that CFCs can destroy the ozone in the stratosphere. In June 1975, the Natural Resources Defence Council (NRDC) sued the Consumers Product Safety Commission for a band of CFCs used in aerosol spray cans. United States's fifth largest manufacturers of aerosol sprays announced that they will reduce the amount of CFCs used in there products. But as things started to get better, The Consumer Product Safety Commission rejected NRDC's law suit in July stating that there was insufficient evidence towards the amount of harm the CFCs were doing to the ozone layer. On September 1976, a report was released which re-enforced Rowland and Molina's hypothesis, but also stated that the govern- ment action on CFC regulations should b e postponed.

Nursing Case Studies on COPD

In this reflective piece of writing I will be explaining how chronic obstructive pulmonary disease (COPD) affects the patient physically, psychologically ,and socially ,I will also explain how the disease affects his daily routine and how it impacts on his family life. I will give an overview of the clinical signs and symptoms, how the disease alters the pathphysiology of the lungs, and what these changes cause within the body. I will be using the reflective model â€Å"What, So What, Now What â€Å" (2007). The patient I have chosen to write about is a seventy year old male who has been married for nearly fifty years. He has two grown up sons, both married with children of their own. Mr Woods has Chronic Obstructive Pulmonary Disease diagnosed ten years ago. Prior to this disease Mr Woods was a lifelong smoker, beginning at the age of fourteen years, smoking up to thirty cigarettes per day. Mr Woods condition has progressively worsened over the past few years, and he now requires home oxygen therapy. A patient was brought into the emergency department by the paramedics complaining of difficulty in breathing. On arrival he was tachypnoeic, had a respiratory rate thirty two and was found to have an audible wheeze. He stated that he had a productive cough and was expectorating green coloured sputum. The patient felt warm to touch. He looked pale, was sat upright, slightly leaning forward in a rigid posture on the ambulance stretcher. I was delegated the role of undertaking Mr Wood’s initial assessment, which included ensuring the patient was undressed ready for examination by a doctor, and also carrying out a baseline set of observations. I was happy to undertake this task, because I had the required training, skills and was deemed competent to carry out the necessary care required to look after Mr Woods. The nurse in charge informed me of Mr Woods medical history prior to me entering the cubicle, including what had precipitated his attendance to the emergency department which on that particular day had been his worsening shortness of breath. On entering the cubicle, I helped Mr Woods get undressed and into a hospital gown because any slight exertion made him more short of breath. I carried out a baseline set of observations. His blood pressure was 165/95, he had a pulse rate of 125 beats per minute, a temperature of 38. c, a respiratory rate of 32, on 2 litres of oxygen his saturation level was 88%, and his blood glucose level was 4. 4mmol/l. Although some of these observations are not within ‘normal’ range, for a person with COPD some of these observations maybe acceptable because the disease affects the path physiology of the lungs. The airways leading to the lungs, the bronchi, become inflamed. The inflamed airways produce too much mucus (sputum) which can lead to a p ersistent cough, wheeze and increasing shortness of breath. This happens because the air sacs (alveoli) become overstretched, rupture and merge which causes them to lose their elasticity. This causes the oxygen absorbing surfaces to be reduced, and with the narrowing of the airways gas exchange is less efficient (Parker, 2009). The lungs over inflate which reduces the air volume moving in and out of the lungs which can lead to tachypnoea (abnormally rapid rate of breathing), breathlessness on exertion, respiratory distress, abnormal posture I. e. leaning forward to help open the airways (Nursing Standard, 2001). Patients with Chronic Obstructive Pulmonary Disease can have a tendency to have low oxygen saturation levels, usually around 88% on air. In healthy patients their levels are usually between 95%- 100%. COPD patients often need supportive treatment of 2 litres of oxygen to maintain oxygen saturations normally acceptable for that specific patient. However oxygen therapy higher than 2 litres may cause their carbon dioxide (CO2) levels to rise (Abrahams, 2009). As Mr Woods COPD had progressively worsened he had been commenced on home oxygen which he uses throughout most of the day. This helps him to undertake the most simplistic of daily activities of living. Mr Woods lives at home with his wife who, due to the impact of this disease on Mr Woods, has now become his main carer. She helps her husband with his daily activities such as washing, showering and preparing his meals. He needs help mobilising to the downstairs shower room, and, once there, needs assistance to get undressed. Whilst in the shower room Mr Woods needs to sit on a shower stool because he cannot manage to stand for any length of time due to breathlessness. He is also unable to walk upstairs because he gets short of breath on exertion so he has had a stair lift installed which enables him to go upstairs to bed. This enables Mr and Mrs Woods to fulfil both the physical and psychological aspects of their relationship. COPD can affect the psychological wellbeing of the sufferer. Before Mr Woods condition deteriorated he was able to go out, he used to enjoy going fishing with his sons and playing with his grandchildren. Because of his condition, Mr Woods is prone to feelings of inadequacy and depression. He also feels guilty because of his growing dependency on his wife for the simplest of daily tasks such as making a cup of tea or answering the door. Because of the growing demands of her husband’s worsening condition Mrs Woods now has to depend on other family members to facilitate her with tasks that Mr Woods can no longer undertake due to his COPD, and frequent visits to the hospital with recurrent chest infections. Whilst Mr Woods was in the emergency department it was my responsibility to make sure Mr Woods was comfortable and that his observations were done regularly and documented. I was happy to do this as I am deemed competent and have the required training to carry out these duties. I made sure Mr Woods was sitting upright as this would help him with his breathing by improving his lung capacity and making sure oxygen was prescribed by the doctor and administered via nasal cannulae as per trust policy. I noticed Mr Woods remained tachyponeic, so repeated his observations. Even though on 2 litres of oxygen his saturation levels still remained low so I informed the nurse in charge that Mr Woods observations remained unstable. A doctor was notified and the patient assessed which involved listening to his chest. The doctor then prescribed nebulisers, oral steroids and paracetamol. A chest x-ray was also requested. I had to ask a qualified member of staff to administer Mr Woods medication, because I am not qualified to dispense drugs to a patient as a student assistant practitioner as this does not fall within my scope of practice or within the boundaries of my role. I think the fact I know my limitations and boundaries make me a safe practitioner. I made sure I had documented Mr Woods observations and that I had informed the nurse in charge of his condition making sure that I had dated, timed and signed what I had written. I got my documentation countersigned by a qualified member of staff as stated by the Nursing and Midwifery Council (NMC, 2008). The doctor asked me to cannulate and take some blood from Mr Woods. I was comfortable with this request because I am qualified to undertake the task. I explained to Mr Woods that I needed to put a needle in his arm and take some blood and that I would be leaving the cannula in his arm for any medication his may require later. I put the equipment together that I needed to cannulate, making sure that it was on a clean trolley and that I had a sharps bin. I then washed my hands, put on my apron and gloves following universal precautions. I then proceeded to cannulate Mr Woods explaining everything I was doing throughout the procedure. Once the cannula was in I disposed of my sharp in the sharps bin and put my dirty equipment in the clinical waste and then washed my hands. Mr Woods was then taken for his x-ray. Once labelled I then gave the blood to the doctor to send of to the path lab. I then filled in the cannulation documentation form as per hospital trust policy. I was pleased that I managed to get the cannula in on my first attempt because Mr Woods had terrible veins and I did not like the thought of having to put him through the procedure again as it can be quite painful and distressing. Mr Woods x-ray showed he had a chest infection for which he was prescribed intravenous antibiotics. Mr Woods was then transferred to the Medical Assessment Unit for further treatment by the medical doctors. Reflecting back I believe I have developed my knowledge about chronic obstructive pulmonary disease. Reading articles, text books and trust policies on COPD has allowed my to enhance my ability to recognise when patients are clinically unwell and have the confidence to highlight these abnormalities to the relevant members of the multidisciplinary team I. e. the nurse in charge and doctor, so the patient can be managed promptly and appropriately. Also the information I gained from talking to Mr Woods was invaluable in allowing me to gain insight and therefore a greater understanding of how the disease affected not only the patient but also his family on a day to day basis. Witnessing first hand the debilitating affects the disease process has on an individual such as Mr Woods and his family left me feeling a little sad due to the fact that my role as a student assistant practitioner limited my involvement in his treatment. Having been the first member of staff to attend to Mr Woods on his arrival to the department and to have spent time developing a therapeutic relationship with him I felt that involving another member of staff to carry out an aspect of care may make him question my abilities to look after him as I could not administer his medication. I could address this issue by explaining to the patient that my role as student assistant practitioner does not allow me to give medication but explain that I am competent in carrying out all other aspects of care. Developing my existing knowledge on the psychological and physiological affects of Chronic Obstructive Pulmonary Disease has been consolidated by caring for a patient that has attended the emergency department with this chronic long term condition. Extending my knowledge base on this condition and the long term effects it can have on the individual will ensure that I treat each patient on their needs rather than just on their condition. Also looking back on this assessment I believe I acted professionally, promptly and efficiently. I feel I carried out my duties to a high standard of care within the boundaries of my role as a student assistant practitioner which in turn enabled Mr Woods to receive the treatment and medication he required to ensure the best possible outcome. Looking after Mr Woods has shown that I can work effectively as a member of the multidisciplinary team. I am able to assess, implement and evaluate my care which has enabled other members of staff to witness my holistic and high level of care delivery within the emergency department. I believe this can benefit not only the patients attending the department but also help develop my role within the team.

The independency of a woman is shown through the main character in this story. The main problem in this short story discusses about Alison that isdescribed as a teenage girl who is lesbian to Laura, her best friend.It will be explained in the textual evidences below. â€Å" No, that’s first. Frist am going to say that there was no need to announce it. You think I don’t know what’s going on with you and Laura? I don’t need eyes in the back of my head to figure that out†. â€Å"I can’t do anything about it, can I? I see it ev ery time you bring her here. I would like to believe it’s a stage you’re going trhought, but from what I’ve read and heard it isn’t†. The certain conversations above are the speech that are told by Alison’s mother that indicate that Alison is a lesbian.It’s also supported by the speech of Alison Grand mother. We can see on the textual evidance below. â€Å"So you don’t have to tell me about what it feels like to be an outsider. You don’t have to tell me about prejudice. But, Alison, I thank you for telling me about your self. I’m proud that you told me first. † Both of speech acts told and emphasize implicitly about Alison identity as a lesbian. As the explanations before that in this story Alison represent the independency of a woman, Alison decides to choose her own way to be a lesbian.It is shown in the conversation between she and her mother. We can see it from the Alison speech trough the conversation b etween she and her mother. â€Å" mom it’s not a choice. Was is a choise when you felt in love with Dad? † â€Å"most definetly i chose him! † We can see that Alisson starts to chalange her mother about a logical opinion about love. Lets see the other interesting on the conversation below. â€Å" So what if the world was different, and men loved men and women loved women, but where still you? What would you do?My mother shrugged. â€Å"Find another world, I guess. † â€Å"So that’s what I did. I found another World†. From the speech from Alisson above it obviously shows the independency of her to make a descion to her own life. She says that words to her mother explicitly. For me, that speech is the most powerful evidence of Alison, and it indicates the independency of her. From the explanation above i bravely conclude that the potray Alison in this short story represents the independency of women to decide and to choose what women really w ant to do.
